How we handle website information.

Information you choose to provide

The general contact form collects your name, phone number, email address, topic, and message. The appointment-request form collects your contact details, service interest, scheduling preferences, and an optional short message. Wellness Dental Hygiene uses this information only to respond to your inquiry or arrange an appointment.

The medical-history form is available only through a direct link provided by clinic staff. It collects identification and contact details, dental and medical history, medications, allergies, health conditions, consent choices, and a typed signature so the clinic can prepare for and provide safe care.

Please do not include detailed medical or dental history in the general contact or appointment-request forms.

Form processing

The contact, appointment-request, and medical-history forms are processed by Formspree, a service provider used to deliver submissions to Patricia at Wellness Dental Hygiene. Information submitted through these forms may be processed outside Canada, including in the United States. Formspree's submission archive is disabled for the medical-history form.

If you do not want to send personal or health information online, call 613.319.3713 to arrange another completion method. General inquiries may also be sent to wellnessdentalhygiene@gmail.com.

Use, access, and retention

Website submissions are delivered to the clinic email account and are accessible only to people who need them to respond or provide care on behalf of the clinic. The clinic retains information only as long as needed for the inquiry, appointment administration, legal requirements, or continuity of care. You may ask to access or correct information you submitted by contacting the clinic.

Website services

This website does not currently use advertising or analytics trackers. The contact page includes an embedded Google map; Google may receive technical information such as your IP address when that map loads. Links to telephone, email, maps, or other websites are handled by the services you choose to open.

Safeguards and questions

Reasonable administrative and technical safeguards are used to protect website submissions. No internet transmission is completely risk-free, so please use the telephone for urgent or especially sensitive matters.
